cleanup for many headers

Hello!

include/*.h and include/*/*.h sync with glibc, disabling all non-provided 
prototypes, this patch will conflict in unistd.h with the earlier sent 
local_lim/posix_opt patch (easier to apply this first and remove the 
unistd.h hunk in the other patch, one-liner)
